The needs of clinicians predominate throughout the text, but these needs overlap with those of researchers especially in chapters covering randomized controlled trials. For readers to assess trials accurately they need to understand relevant guidelines on the conduct of trials that are emerging from methodological research. In presenting these discussions to clinicians these chapters will help researchers who also do randomized trials and provide a methodological background that enhances the quality and quantity of their research productivity.

Margaret Chan, Director General of the World Health Organization, has stated that "Universal Health Coverage is the single most powerful concept that public health has to offer." In this Series from The Lancet, the first of three papers explores the evidence on the links between expansions in coverage and population health outcomes. The second paper looks at the political and economic dimensions of the transition to universal health coverage, and the third examines nine low-income and lower-middle-income countries in Africa and Asia that have implemented national health insurance reforms. Also, a Viewpoint calls for continued progress and argues for a large public sector role in health systems reform. What becomes clear in this issue is that although universal health coverage is not a guarantee for progress, attention should focus now not on whether, but on how to make the most of the transition. In a Comment, Judith Rodin and David de Ferranti conclude: "what will emerge in the decades ahead in each country undertaking reform is not entirely clear, but, as this Series and many country examples suggest, we are getting closer to a time when this [transition toward universal health coverage] will be achieved and families will no longer be at risk of having the cost of sickness ruin their lives." The Lancet Global Health Series are groups of articles on a topic of global health importance. Series papers and Comments, related articles, links to relevant publications, media highlights, and audio interviews are available from http://www.thelancet.com/global-health-series. Research into prevalent conditions and wider health policy issues are reported by a range of experts.

Collaboration between The Lancet and Imperial College London, UK, has resulted in a new Commission, which examines how medical technology should best be used to improve health in low- and middle-income countries. The report concludes that in many cases, medical technology—almost exclusively developed in rich countries—is simply inappropriate for use in poorer nations.

Around 2·6 million stillbirths (the death of a baby at 28 weeks’ gestation or more) occur each year. Although 98% of these deaths take place in low-income and middle-income countries, stillbirths also continue to affect wealthier nations, with around 1 in every 300 babies stillborn in high-income countries. This book highlights the rates and causes of stillbirth globally, explores cost-effective interventions to prevent stillbirths (as well as maternal and neonatal deaths), and sets key actions to halve stillbirth rates by 2020. Also included are Comments from professional organisations and parent groups, the latter demonstrating the unique tragedy for families of the birth of a baby bearing no signs of life. This research was launched in London, New York, Hobart, Geneva, New Delhi, Florence, and Cape Town on April 14, 2011.
